Filing a comment or complaint about a
USC Approved Assembly

The continuing USC Approval status of an assembly is contingent upon ongoing satisfactory field performance. If you have experienced a problem (i.e., other than routine repair and maintenance issues) with a backflow prevention assembly, and would like to bring this to the attention of the Foundation, please e-mail (fccchrlab@usc.edu) or call (323-442-0470) the USC Foundation laboratory with the following information:

  1. Make, model, size and serial number(s) of the assembly.
  2. Pertinent operating conditions such as line pressure or flow conditions (i.e., continuous/intermittent) may be helpful too.
  3. Provide a detailed description of the problem.
  4. Photographs of the issue may be helpful.
  5. If actual components are available for submittal, they may be shipped to our laboratory at:

Foundation Staff will acknowledge receipt of the submitted information, and may contact you for additional information (i.e., specific details or possibly sample components). Please note, the USC Foundation will maintain your submittal in a confidential manner. Any correspondence with a product manufacturer will take place between the Foundation and the manufacturer's representative(s). The Foundation Staff will start an investigation of the issue and depending upon the findings of the Foundation's independent investigation, the Foundation will work with the manufacturer for a resolution of the issue. You will be notified of the Foundation's findings, and any corrective actions that may be warranted.
